Announcement: About our founder, Thomas Ortiz, MD, FAAFP A board certified Family Physician, Dr. Ortiz has been in the private practice of Family Medicine as the Founder and Medical Director of Forest Hill Family Health Associates, PA in Newark, NJ since 1984. Building on experience, in 2010 we were nationally recognized by NCQA as a Patient Centered Medical Home and maintain this recognition currently. A graduate of Rutgers NJMS, as a public health professional, Dr. Ortiz has successfully advocated for access to Primary Care and Health Care justice for the poor, the underserved and the disenfranchised. A long time champion for health care reform, Dr. Ortiz has served on many local and statewide task forces, commissions and committees, representing Primary Care medicine and Health Information Technology, receiving peer recognition awards community service awards and accolades. Dr. Ortiz continues to lead for Primary Care infrastructure development, clinical team integration, payment reform, technology innovation, interoperability and an advocate for Patient Centered Medical Home (PCMH) principles for health care delivery. Leading MDs and other NJ health care stake holders, Dr. Ortiz served as past president and past chairman of the NJ Academy of Family Physicians, representing over 1800 FPs and other PCPs in NJ. He formally served as the Consultant Medical Director for NJ Innovations Institute- Garden Transformation Network at NJIT. In community service, Dr. Ortiz currently serves as Medical Director for the Center of Excellence for Latino Health at Clara Maass Medical Center. He has formally served on the Board of Trustees, Executive Committee as the chairman of the Population Care Committee of the Greater Newark Health Care Coalition. I am able continue my career, by the Grace of God, as a Family Physician and Medical Director at Forest Hill Medical Arts Center, serving the diverse Greater Newark community, providing high quality primary and specialty care services, in the same location, for over 40 years. AZZ Medical Associates, a group of NJ physicians, is now our partner management company and is supporting our medical team and support staff, as well as facilitating patient doctor communications utilizing advanced technologies.

Directing the efforts of the team at the medical center to outreach and provide appropriate and culturally competent medical and surgical services for the Latino community in the region. This will include social service and health education outreach and services as well as addressing the specific identifiable medical needs of the Latino community including behavioral health, obesity, diabetes and hypertension.
