Resident Care Coordinator Experience ➢ Reporting directly to the Executive Director who manages the overall operations for the company. ➢ Plans, organizes, develops, and leads the overall operation of Resident Care for at least 80 residents in accordance with federal, provincial, and local laws, company standards, guidelines, and regulations. ➢ Prior to move-in, reviews the resident's file and schedules the Family Meeting to discuss Resident Profile; coordinates the health care needs with the Resident Care Director; ensures resident has current Individualized Service Plan according to companies policy and government guidelines. ➢ Manages the assessment process to determine levels of care with the appropriate staffing levels. ➢ Partners with the activities and volunteer coordinator to ensure a variety of appropriate and individualized activities are available through the day and evening and that care managers are actively involved and engaged with activities. ➢ Partners with the dining service coordinator to schedule and plan special events; monitors dining experience daily. ➢ Understand the recognition of resident changes in condition and takes appropriate action. ➢ Communicates regularly with families and physicians as appropriate. ➢ Ensures that the community follows all federal, provincial, and local laws and regulations and company standards as it pertains to resident care and services. ➢ Develops and implements appropriate plans of action to correct identified deficiencies in compliance with company expectations for Quality Service Reviews and other regulatory compliance. ➢ Develops a thorough working knowledge of current and evolving provincial laws and regulations, policies and procedures dictated for residents and ensure compliance. ➢ Partners with leadership team to ensure community is in compliance with OSHA requirements. ➢ Practices safety procedures at all times including (PPE), fire extinguishers, MSDS and Lockout Tagout procedures.
