I am a highly dedicated, professional Doctorate in Physical Therapy. I have a variety of experiences in a variety of settings including but not limited to: clinical management, home health care, inpatient rehabilitation, acute care, vestibular rehab as well as lymphedema management and oncology. I am dedicated to the profession of Physical Therapy and my education, which is why I will always strive to improve myself as a professional health care provider.
-Act as the liaison to the patient and patient’s PCP (Primary Care Provider), and works in partnership with clinical and administrative staff. -Facilitates multidisciplinary case conference meetings with clinical and administrative staff to coordinate consistent delivery of patient care. -Oversees the submission and follow-up of verbal orders, supplemental orders, authorizations, re-certifications, or additional disciplines, in coordination with the PCP. -Oversees the work of schedulers, field therapists, and other administrative office staff. -Identifies opportunities to enhance efficiency. -Manages performance and develops solutions to enhance skills to determine staff training needs.
-Performs evaluations of mobility, strength and level of function to establish goals related to improving independence and safety within the home setting. -Performs assessments of home environment to recommend proper equipment and safety education in order to prevent hospital readmission. -Participates in care coordination with other health care disciplines ( occupational therapy, social work, speech therapy and nursing) in order to provide the best outcomes and patient satisfaction.
