Coordinates and leads a team of professionals in medical record review and provider education activities related to clinical quality measures (HEDIS, GPRO). Performs chart pulls and medical record preparation to submit for gap closure to health plans. Identifies best practice opportunities and drives quality improvement efforts at the practice level. Participates in and supports ACO performance through data analytics, project management, and staff coordination. Pulls and processes clinic quality data from payer datasources.
