Below are just a few responsibilities that Jewelyn provides: * Conducts & coordinates an initial comprehensive assessment by a certified Aging Life Care Manager * Assist family with providing caregivers from homemakers to certified aides with specialities in aging * Monitors & checks in periodically (weekly, monthly, or more often as requested) * Makes recommendations for alternative living arrangements, if needed * Maintain contact with all family members to keep them abreast of their loved-one and their care * Attend medical appt. to assure recommendations are noted & followed by your loved one * Provide V.I.P. care coordination, management, & special touches to assure you are receiving the Premier Elder Care services.
