I conduct trend analyses on all patient safety events and root cause analyses on events with a moderate-to-high probability of reoccurrence. I also prepare slides and present briefings on qualitative/quantitative data during 12 scheduled Executive Leadership meetings. •I am a certified Master TeamSTEPPS Instructor. I instruct all newly hired members on TeamSTEPPS Fundamentals within 90 days of in-processing. Within the first 8 months of hire, I increased Initial TeamSTEPPS training 12% and Annual TeamSTEPPS Training 96%. Highest rate in 3 years! Monthly TeamSTEPPS training compliance remains at 99%. •Through facility rounds, patient questionnaires, 55 Patient Tracers, and 110-item biannual self-assessments, I validate compliance for The Joint Commission Survey. When non-compliant items are identified, using Lean principles, I team with department heads to pinpoint causal factors, develop countermeasures, track progress in TJC AMP and brief updates to Executive Leaders in monthly Compliance Meetings. I led an 18-month long, 9-member Proactive Risk Assessment targeted at reducing prescription errors. My team identified 32 specific error types being made by 20 provider teams.
