• Provides individual and couple therapeutic services to assist individuals and couples in achieving a more effective personal, social, educational, and vocational development and adjustment. • Conducts regular therapy with the patient. • Communicates with patients’ family members regarding ways they can participate and help with patients’ treatment plans. • Provides pertinent information about patients to providers, psychiatrists, nurses and other disciplines who participate in the patient's’ treatment plan. • Monitors treatment processes and makes changes when necessary. • Refers patients to institutions, support services, and specialist as needed. • Maintains confidentiality of information at all times. • Complies with applicable policies and procedures and supports the Compliance Program. • Contributes to organizational operations by performing other duties as may be required.
Social Worker •Provide individual assessment, treatment planning, and psychotherapy services to socio- culturally diverse older adult population . •Initiate and collaborate on creating a physicians program to provide psycho therapeutic care in physician’s offices. •Refer clients to community resources and coordinated services with collaterals. •Collaborate with interdisciplinary teams for planning and implementing patient care. •Maintain documentation for records and insurance.
