Worked in a Care Plan Note team and my role included preparation of psychiatry notes for the providers based in the USA. Transcribed the entire handwritten patient's health data into the Electronic Health Record (EHR) database using Gehrimed software as per each provider's requirements before the deadline with zero errors. Determined patient's psychiatric conditions through HPI and treatment plans, summarized it. Updating patient's medications, vitals, MSE and labs. Able to prepare psychiatry New evaluation and Care Plan note with the help of GDR sheets of more than 30 providers. Has knowledge of psychiatric conditions, symptoms, diagnosis, medications, Gehrimed, CPT and Dx codes.
