I am clinical nurse lead of a community diabetes service, driving innovation and equity in diabetes care. With a strong foundation in advanced physical health practice and independent nurse prescribing, I specialize in managing complex diabetes cases while championing holistic, patient-centered care. My work spans across service transformation, quality improvement, developing and supporting nurses and strategic project management. I’ve led the development of digital tools to enhance clinical outcomes and patient engagement, and pioneered new roles within the service — notably establishing the Health and Wellbeing Coach team to support lifestyle change and self-management. Through targeted community outreach, I’ve expanded access to diabetes care, particularly among underserved populations, aligning with my deep commitment to tackling health inequalities and advancing public health. I’m passionate about frailty and its intersection with long-term conditions, and I advocate for integrated approaches that address both medical and social determinants of health. Triaging New referrals in diabetes care. Involved in doing consultation for new and follow-up clinic in diabetes care. Involved in diabetes administration e.g clinic letters and making referrals. Providing support for district nurses and other healthcare professional. Proving safe clinical advice in diabetes care to GPs and other Healthcare professionals. Providing teaching and supervision to Healthcare professional. Managing complexities in diabetes care e.g hypoglycaemia, hyperglycaemia, palliative care and pef feeds. Involved in innovations in community diabetes care.
