Case manager and service supervisor in Integrated Home Care Support Service
1. Case management and counseling -To provide counseling support for the hidden elderly and the carers in the community -To manage long-term caring plan with the needy elderlies and their carers, including those with Dementia -To conduct MDS-HC assessment 2. Groups and programme -To conduct Narrative groups for the carers -To conduct Support groups for the Hidden elderly -To conduct mindfulness group for the elderly with Depression 3. Community work -To coordinate with Housing Society and regional elderly centres in regional works for Hidden and Vulnerable elderlies -To conduct community outreach visit on regular basis for promoting the elderly service and outreaching potential hidden elderlies -To coordinate the service cooperation with partner church in providing spiritual support to the elderly 4. Digital development of the elderly service -To conduct regular video programme through Youtube Channel
1. Service planning -To coordinate the development of Hidden and Vulnerable Elderly Service by outreaching the potential hidden elderlies and planning for relevant support service 2. Case management and counseling -To provide counseling support for the hidden elderly cases in the community -To manage long-term caring plan with the needy elderlies and their carers, including those with Dementia -To conduct MDS-HC assessment 3. Groups and programme -To conduct educational groups of Dementia for the carers -To conduct Men’s developmental group 4. Volunteer development -To establish and develop community-based volunteer hairstylist team -To manage and develop the Chinese Orchestra team -To provide volunteer training by enriching the knowledge of the volunteers on the needs of hidden elderlies and elderlies with Dementia 5. Community work -To coordinate with regional elderly centres in regional works for Hidden and Vulnerable elderlies -To conduct community outreach visit on regular basis for promoting the elderly service and outreaching potential hidden elderlies -To coordinate the service cooperation with partner church in providing spiritual support to the elderly
Core duties 1. Case management and counseling -Provide counseling support to the clients with mental health, personal trauma, parenting and complex family issues (Eg. Family Violence, Elderly Abuse) - Provide financial coaching to the clients with financial needs -Monitor and manage the clients with complex family issues and the related risks (Suicidal and family violence risk) 2. Group and Programme - Plan and implement the therapeutic group with children, youth and single mother for equipping them with emotional resiliency in handling changes in divorced family -Plan and implement the educational programme with children for character building 3. Community Work -Liaise with external agencies, including school social worker, Social Protection Officers and medical social workers, in order to collaborate in service delivery -To liaise with Member of Parliament in Meet the People session
