Manage daily operations for 62 bed SNF. Short term rehabilitation with Full-Time Physical, Occupational, and Speech Therapy. Provide long term care for custodial residents with chronic diseases. Developed pilot program in coordination with Alameda County Behavioral Health Services to provide specialized care for residents with chronic psychiatric needs in SNF setting. Developed and implemented comprehensive quality assurance program, inclusive of standardized care standards. Maintained excellent compliance history with CMS and CDPH.
