Responsible for managing service coordination for a designated caseload in specialized programs. Collaborate with individuals to lead the Person Centered Planning process, documenting their preferences, needs, and goals. Conduct assessments, create comprehensive Person Centered Care Plans (PCCP), and develop backup plans. Work with Medical Directors and partake in interdisciplinary care rounds to establish a fully integrated care plan. Engage the individual's support network and oversee management of their physical health, behavioral health, and long-term services and supports, adhering to state and federal regulations.
Conduct comprehensive screens, needs assessments and psychosocial evaluations for diagnostic purposes. Provide substance abuse counseling to adults and adolescents in group and individual settings. Monitor client progress and adherence to treatment plan; maintain client files and case documentation in accordance with related policies. Provide alcohol, drug, and tobacco prevention/education to various schools, social service agencies, juvenile drug treatment court and community organizations. Provide case management for adolescent and adult clients in treatment; linking to available resource to address barriers and social determinants of health.
Coordinate and manage youth and adult anti-tobacco programs and assist in grant writing to support programs. Provide valuable interventions for tobacco dependence which addresses addiction, nicotine withdrawal symptoms, coping skills and effective treatments for tobacco cessation. Promote education of the community, hospital personnel, worksites and schools regarding tobacco free policies. Present to local groups and organizations to educate and establish a rapport between members of the community and the tobacco education program, to publicize available services, and to assist in resolving issues facing the communities regarding tobacco use/vaping, abuse, and associated health issues.
