Clinical pharmacist specialist for the Community Living Center (CLC) which includes geropsych, rehab, palliative care, and skilled nursing wards for 100+ beds. Duties include monthly medication regimen reviews, community nursing home medication billing audits, admission and discharge medication reconciliation, weekly infectious disease rounds, weekly endocrine rounds, biweekly geropsych rounds, pharmacokinetic monitoring (vancomycin). Cross-trained in inpatient operations/distribution, emergency room, as clinical pharmacist for oncology.
Per diem long-term care pharmacist responsible for quarterly policy and procedures report and comprehensive drug regimen reviews for 70+ intermediate care facilities. Additionally, per diem community pharmacist, staffing retail pharmacy as needed.
Primary job responsibilities in transition of care/discharge including comprehensive medication reconciliation, discharge counseling, and distribution/operations of the discharge pharmacy. Participated daily at internal medicine rounds for medication optimization and intervention. Cross-training experience included: emergency room, cardiology, outpatient chemotherapy and specialty medication infusion clinic, and inpatient operations/distribution.
Residency training in the following areas: oncology (focused on outpatient IV infusion chemotherapy), formulary management, primary care medication titration clinic, inpatient and outpatient infectious disease, transitional care, internal medicine, inpatient and outpatient geriatric medicine, inpatient operations, and inpatient and outpatient anticoagulation Longitudinal Work: Inpatient operations and transitional care staffing every other weekend (16 hours/month). Responsible for presenting monthly local and national VA adverse reactions reports to P&T Committee and Safe Medication Use Committees. Patient-aligned Care Team Titration Clinic: Weekly clinic with 7-9 scheduled telephone and face-to-face appointments where patients were assessed, medication regimens optimized, and provided therapeutic lifestyle education for patients with uncontrolled diabetes, hypertension, and cholesterol (8 hours/week).
Provide patient consultations, administer immunizations, engage in blood pressure screenings, coordinate with prescribers to optimize patient drug therapy, and fill prescriptions.
