•Provide Assessment, treatment, discharge and coordinate a clinical treatment program for acute or chronically mentally ill client •Collaborate with treating physician, psychiatric and allied health professional team to plan and direct each individual client’s treatment program and discharge plan. •Provide consultation to psychiatrists, allied health professionals, and other specialty care providers on psychological assessment. •Consult with staff regarding diagnosis, strengths and deficits of client, as needed or appropriate. •Chart client's treatments and progress in accordance with state regulations and department procedures. •Interpret psychiatric treatment to client's family and help to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment. •Make referrals to other case managers, as appropriate, and/or refer clients to community support services and resources. •Present the client cases in front of county meetings stating the reasons for client requiring mental health services. •Facilitating groups such as Recreation group, coping skills and Relaxation groups. •Drafting and filing the legal paperwork such as 5250 hold, voluntary hold and conservatorship.
•Assisted at the front desk with filing paper work, answering phone calls and entering doctors orders into the system •Assisted with discharge planning/ family meetings/aftercare planning •Evaluating Mini Mental Status as well as Geriatric Depression Scale on Senior Patients •Taking patients vital signs, observations during mealtime, maintaining patients adls and also working with patient on 1:1 based on assigned •Facilitating groups such as Psycho educational groups, Recreation groups, Community meeting, Relaxation groups, Relapse prevention, CBT Concepts, and Addiction support groups •Initiating, observing, and documenting needs of the patient in order to ensure the safety on the unit.
•Generate and distribute Press Ganey reports and graphs/data to different department managers •Responsible for preparing written correspondence; addressing patient concerns/complaints both in person and via telephone •Providing assistance to the many departments and activities supported by the customer satisfaction department •Coordinating meetings and recording minutes for the monthly hospital meeting •Proficiency in Microsoft Excel, Word, PowerPoint, outlook and STAR. •Planning and organizing workload by setting priorities •Strong communication skills
