- Provided end-of-life care and bed-side nursing at licensed six-bed residential care facility for patients with life expectancy of six months or less, who have determined to pursue comfort and quality of life. - Educated patients and their families on disease processes, symptom control, pain management and psychological support. - Assisted with development and implementation of facility nursing policy and procedure, patient admissions and emergency planning.
- Managed a panel of 60 patients, identifying gaps in care, preventive services, identifying suboptimal lab values, needed prescriptions, and chronic condition management, including appropriate testing and referral to specialty clinics. Consulted with members of all specialties including endocrine, cardiology, hematology, social work, nutrition, pharmacy to develop treatment plans. - Patients were medically complex veterans aged 22-98, often with several comorbidities, including PTSD, substance abuse and homelessness. - Provided team based care involving an RN, LVN, clerical associate, medical residents, clinical psychology fellows and psychiatry residents.
- Completed 525 hours in Medical-Surgical Nursing in Oncology Surgery Unit. - Assessed patients, implemented medical interventions, including administering and documenting scheduled medications, anticipated potential complications, and evaluated care outcomes.
