First to initiate the (CTE) Clinical Transition Educator Role in the State of Washington, then ID, OR, & Utah. Training, Mentoring, and Leading new RN's in this role in: WA, OR, Idaho, and Utah. Role: 1. Coordinate with CTS's daily to Drive to and Teach their Patients (IV abx, TPN, or Enteral Therapies) at various hospitals in the Seattle/Everett/Tacoma Territory 2. Teach patients, Complete Documentation per OCH, Collect Co-Pays/Self-Pays during Training, and work with RN Scheduler to schedule 1st Nurse Appt during Teach. 3. Cover the CTS Role during their PTO (which entails learning how to receive/request referrals from CM's, RD's, and MD's, along w/ knowing the Charting System at each of their assigned Hospitals and driving all over the state of Washington to accomplish this; see full details in CTS position below).
As a (CTS) Clinical Transition Specialists, we are experienced Nurse sales professionals who utilize sales techniques to sell Option Care products and services to discharging patients. As a CTS, we are responsible for proactively building strong relationships with referral sources while also partnering with Account Managers to grow referral rates and achieve sales goals. We are responsible for educating patients, their families, and the facility staff about how the services and products will be facilitated at an alternative site. CTS's ensure proper placement of patients within the Home Health Care setting by assessing patients, gathering preadmission information, collaborating with internal staff (intake) and external staff (Case Managers, Discharge Planners, Infectious Disease Doctors) partners to ensure quality of service and implementation of an effective Treatment Plan. Patients appreciate being in their own bed in their home environment with family and loved ones; and, the Insurance companies spend less money than a patient at the hospital or a SNF. Telemetry, Trauma, ER (boarders), ICU(step down), Progressive Care, Orthopedics, Oncology, and Med/Surg
* Supervise and Manage a team of 45-50 RN's, LPN's, and CNA's per shift in the facility during night hours to ensure proper and timely patient/resident care. * Ensure proper staffing-per-unit at the beginning of the shift. * Listen and rectify all patient/resident complaints * Assess and triage patients/residents when out of normal range - facilitate transport to hospital if deemed necessary * Assist with new Admissions: ensure all MD orders are input accurately into PointClickCare EMR, ensure Admission forms and Progress Notes are complete, and start preliminary Care Plans. * Administration of medication including IV meds, Respiratory treatments, and wound care with evaluations of effectiveness * Care, feedings, unclogging, and changing of: Tracheostomy tubes, Gastrostomy tubes, Percutaneous Endoscopic tubes, and Foley Catheters * Obtain supplies from Central Supply for staff needs
Telemetry RN, but, have had the privilege and opportunity to float to the following units, starting in August 2015: PCU (Progressive Care Unit), CDU (Clinical Decision Unit), ED (Emergency Dept), and MedSurg.
