As a member of the HAWCST team, pharmacy technicians would perfom MTMs (medication therapy management) with patients over the phone. Our duties would include surveying our patients about the medications that they are taking and documenting them for a pharmacist to look over, listing any concerns a patient might have about their medications or living conditions, picking up on subtle hints that a patient might be experiencing some troubles related to their living environment, personal health, or ease of access to health care, and then compiling all of this information for our pharmacists to review. As part of our procedures, our technicians would warm transfer our patients to a pharmacist to have them speak with our patients and address any concerns that the patients might have.
