I have been a practising social worker for 19 years. I am currently in a senior clinical/ leadership role in a hospital setting. I am passionate in helping people of all ages in their health care journey. My particular area of expertise is working with older people and people experiencing hoarding and squalor. I work in the rehab subacute space and assist people with complex discharge planning.

● Manage a complex caseload of older adults with multiple health, mental health, and psychosocial needs ● Conduct comprehensive assessments to inform individualised care plans aligned with client goals and funding guidelines ● Coordinate and broker services including personal care, nursing, allied health, respite, and community supports ● Monitor service delivery to ensure quality, safety, and value for clients ● Identify and respond to risk including elder abuse, neglect, self-neglect, and carer stress ● Support clients and families to navigate aged care systems, funding, and service options ● Provide leadership, mentoring, and guidance to case managers and support staff ● Maintain accurate, timely documentation in line with organisational and regulatory requirements
