I am passionate about bringing mental health awareness and treatments to decrease unnecessary mental health suffering for all. I am a clinician and case manager with a University of Southern California’s Master of Social Work degree and specialization in Military under the study in Adult Mental Health and Wellness, who is looking for a career involving the treatment of mental health as an Associate Clinical Social Worker (ASW).
•Developed and ran curriculums for open and closed processing groups of various functioning levels encompassing numerus modalities to increase patient’s adaptive coping skills and peer collaboration for increased management of mental health symptoms. •Conducted psychosocial, quality of life assessment, harm to self and harm to others intervention plans, and safety assessments upon intake and throughout treatment to improve patient care, patient progress, patient compliance, and census towards mental stability and treatment goals. •Assessed patients for safety, symptoms, behaviors, progress of treatment goals, medical necessity, and medication compliance in 1:1 settings by providing through the implementation of multiple interventions including motivational interviewing, CBT, DBT, harm reduction, solution focused therapy. •Collaborated in a multidisciplinary team comprised of administration, psychiatrist, and nurses along with patient’s payees, therapists, doctor, conservators, and/or family to then provide patients with comprehensive case management, resources and referrals, and insurance reauthorization for services. •Provided additional to support to other outpatient locations and inpatient for continuity of patient care. •Maintained and adhered to HIPPA with a strict level of confidentiality and completed accurate clinical documentation using Evident to ensure client protection.
• Guided medically discharged servicemembers and their families in financial stability, whilst providing case management, a place of residence, and support with the intention to decrease the medically discharged homeless Veteran population as they transitioned into the civilian sector. • Conducted psychosocial needs assessments to determine the Transtheoretical Stage of Change and referrals through Motivational Interviewing. • Deescalated patients during difficult events utilizing Crisis Management, SMART goal setting, and therapies, such as Problem-Solving Therapy and Solution-Based Therapy. • Created and lead group therapies in concepts of Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and Wellness Recovery Action Plan, in addition to addressing transitions and self-care amongst not only the servicemember and their families, but also the caregivers. • Appointed acting Director of the San Diego branch wherein I conducted full intake, managed residential concerns and needs, and transitioned servicemember into to the residence.
• Performed case management through psychosocial and needs assessments to service servicemembers, one-on-one both in-person and remotely under the Spinal Cord Injury Department, with resources and healthcare management in both inpatient and outpatient settings through Motivational Interviewing and rapport building. • Coordinated comprehensive care for Veteran patients at the Chula Vista Veteran Home with an interdisciplinary team of nurses, doctors, social workers, and Disabled American Veterans representatives. • Maintained and adhered to HIPPA with a strict level of confidentiality and completed accurate clinical documentation using CPRS to ensure client protection.
