My achievements are based around success stories of my service users, their progress, and their feeling of fulfilment. I inform my practice around Homelessness, Addiction, Mental Health and Complex Trauma, Polydrug Addictions, and holistic approach. Passionate, enthusiastic, creative team worker who tries to inspire people that I support as well as my colleagues. Initiating new projects; Champion of Healthy Working Lives (North Lan South Team Outreach). Involving my colleagues through direct and transparent conversations. I manage my time well, work well unsupervised. My leadership capabilities and my passion are used to improve the lives of service users, patients, carers as well as my own performance. I am instrumental in looking at more creative ways when engaging service users, I have purposeful and solution focused approach however I keep building on my knowledge and understanding around evidence-based interventions. I spend quality time getting to know my patients/carers' needs and aspirations and believe in practicing human relations flexible person-centred approach when with service users as their needs may change from day to day. I recognise the challenges when supporting service users. I am aware of the benefits of using Harm Reduction, Person Centered approach and Motivational Interviewing when supporting service users, meeting service users where they are at. I have experience in supporting service users with Complex Needs and Complex Trauma and I hope to build on my skills, knowledge and experience through researching, personal development, ongoing support, supervision and speaking with colleagues. I keep accurate recording of notes, monitoring, updating “My Safety Plans” when required. I recognise when to take time to reflect on my practice. I work well in collaboration with agencies and authorities. I make every effort to have a positive influence with service users, colleagues, and stakeholders. Before my recent position I personally initiated Community Based Support (this has been evidenced by magazine, newspapers, and radio interviews).
Providing a link between the NHS acute settings and the support provided by community and voluntary organisations. Supporting all NorthLan residents aged 16+ with a presence in both Monklands and Wishaw Hospital as well as other rehabilitation settings throughout Lan to support the discharge of NorthLan patients. Liaising with patients, families, and their carers to offer advice on possible community supports and refer to appropriate organisations when required. Encouraging people and assisting to engage with additional supports with the anticipated outcomes of reducing loneliness and isolation, improving self-management of conditions, encouraging independence and physical activity. Supporting people throughout the patient’s journey, and this can be continued post discharge. Providing information on Local Authority services (such as Social Work, Leisure and Culture) and health promotion literature. Receiving referrals from NHS, our health & social care colleagues, community partners and self-referrals directly from patients and carers. Early links during the discharge planning process allows for the Community Liaison Service Team to build a relationship with the family and having an outcome-based conversation. Helping to identify supports available from voluntary organisations and partners within the Community Solutions programme and across the wider community and voluntary sector. Tailoring information to the patient, family and carers individual needs and interests. Supporting the discharge to assess pathway and providing home visits on their initial return home, ensuring that they are safe to await the scheduled arrival of a colleague from the Community Rehabilitation Team. Targeting Carer Awareness, Equal Partnerships in Care, and the Carers (Scotland) Act 2016 in general, bringing some continuity of carer support between wards.
Complex Needs, Trauma, ACE's, Addictions, Homelessness
Intensive Support Services (Homlessness - Complex Needs) Providing positive practical and emotional support to people who use the service that focuses on goals, promotes choice and encourages personal responsibility. Establishing and maintaining effective professional relationships with a range of external partners to ensure support to service users is coordinated and in agreement with identified care plans. Working with service users to develop and deliver person-centred support plans that achieve the best possible outcomes for people who use the service. Encouraging people who use services to take an active role in their support and in all decisions relating to them or their support. Participating in the monitoring and evaluation of services provided to people who use the service .
Psychotherapist using 'talking therapy' to help clients explore and express their thought processes, feelings and behaviour. Sessions with clients covered range of issues, including: - emotional : such as anger or grief - mental ill health: example anxiety and depression - behavioural issues: eating disorders - ACE - PTSD - Trauma & Complex Needs - addictions During sessions I helped my clients to gain understanding of their inner conflicts and find new ways to alleviate and deal with distress. I' also supported them to make positive changes to the way they think and behave.
As well as actively working within mental health sector I am a successful restaurant leader exemplifying a variety of management skills. Strong manager is an essential component of a successful restaurant handling both the staff and customer issues, passionate about what I do, and determined to succeed. Passion makes things happen.
