A highly motivated registered nurse seeking an opportunity to advance skills in patient advocacy, critical thinking, and communication and provide the highest quality, compassionate care to each individual with the requisite skillset. • Evaluate staffing requirements, floor assignments, and organize quality care for a 66-bed short-term & long-term facility with rehab services. • Supervise staff consisting of 4 RNs/LVNs, 6-8 CNAs and assist with daily duties. • Interpret patient information and make critical decisions about needed actions. • Receive reports from hospital and input admission orders for new patients and initiate care plans. • Communicate with patients, patient’s families, doctors, nurses, CNAs, administrative staff, and all other departments including hospitals, doctor’s offices, pharmacy, laboratory, and any other aspect of patient care to maintain quality patient care.
• Provide direct nursing care to 16 to 25 post-acute and long-term patients. • Provide accurate nursing assessment, over-sight, and monitoring of patients for quality medical management and early detection of changes in condition. • Properly prepare and safely administer medications including controlled substances and IV therapies via oral route, tube feeding, and IV route. • Provide professional nursing services including catheterization, tube feedings, suctioning, IV placement and care, Midline and PICC line care, colostomy care, labs, wound care, including wound vac, and more. • Effectively document skilled nursing assessments, nurse weekly assessments, and other as needed assessments at end of shift. • Encourage patients to participate in prescribed therapies, dietary regimens, and activity programs to facilitate healing. • Collaborate and effectively communicate with healthcare team, including MDs/NPs/PAs, PT/OT, speech therapy, dieticians, CNAs, RNAs, and more to ensure high-quality patient care. • Provide orientation and training to new hires (RNs and LVNs) in facility.
• Manage time effectively through verification of patient eligibility for appropriate services and handling appointment requests. • Provide patient teaching and training on safe handling and use of contact lenses. • Partner with healthcare and administrative staff to create patient-centered, inviting and supportive environment.
• Delivered outstanding, quality patient care through the use of the nursing process for patients with congenital heart defects, heart/lung transplants, cardiovascular surgery, and cardiology patients. • Delivered compassionate care for three assigned patients and documented data related to care, including assessment results, interventions, medications, and patient responses on EPIC. • Collaborated with patients, family, and healthcare professionals to develop an individualized plan of care. • Administered medications and intravenous fluids. • Maintained PICC line care through the use of the evidence-based CLABSI prevention bundle. • Provided effective patient and family teachings related to illnesses and treatments using strong communication skills.
• Assisted doctors with vision therapy and behavioral vision care for pediatric patients with strabismus/amblyopia or learning-related vision problems. • Completed clinical documentation in accordance with guidelines.
