• Establishing and coordinating the ‘Customize Employment’ pilot (Transition to Work) in the South Metro/South West Region – Customize employment: Support to achieve suitable and sustainable employment/work results using a customize employment approach and the discover process. • Promoting all of Intelife’s programs to external stakeholders and linking families within the programs. • Assisting families with establishing short and long term goals. • Assisting in supporting students and their families to navigate and access service options including employment and non-employment options. • Promoting and engaging with schools in the South metro and South West to identify and assist students in transition into Intelife’s programs. • Marketing and presenting to schools, organisations, local government and networks - strong community engagement. • Undertaking initial planning with student and their family in regards to aspirations for post school. • Guiding and supporting students and their families who are entering their last years of school. • Liaising and working with other stakeholders to make appropriate referrals for students leaving school into Intelife’s programs. • Maintain knowledge of relevant trends and issues in relations to the provision of service delivery. • Develop and maintain a network of contacts in the community, through networking, meetings, forums and interagency and other disability organisations. • Promote Intelife services to existing and potential students and school partners at meetings and dedicated events such as school expos. • Meeting KPI’s, deadlines, project targets and reporting. • Advocating for the individual and their family to gain funding. • Sound knowledge of the NDIS, DSC, WANDIS schemes.
Contracted to deliver Transition to Work • Engaging one-on-one with Headspace clients – 12 to 25 years old. • Collaborating with other caseworkers to support clients. • Supporting and assisting clients into education and employment. • Sensitivity and advocating for the client and mental health. • Working with the client to overcome barriers. • Building the clients self-esteem and confidence by delivering professional development activities such as interviewing skills.
• Engaging one-on-one with ‘at-risk’/disengaged young people aged 15 to 21 years old. • Effectively on-going case management of 90+ clients, to build the foundations for a suitable career whilst addressing and overcoming barriers that may affect employment and education. • Negotiating individual mutually agreed Job Plans with clients – 25 hours of participation, whilst motivating and implementing strategies to better employment and education outcomes/personal and professional development. • Understanding complex behaviours and barriers. • Working collaboratively with external stakeholders to assist the client and referring clients to other service providers as appropriate. • Delivering and facilitating educational seminars. • Organising clients to participant in certified training such as the Certificate III in Hospitality. • Recording and reporting full case management notes • Working autonomous and managing the Fremantle site. • Weekly reporting, achieving KPI’s, meeting deadlines and general administrative duties including filing, navigating on ESS and Microsoft Office. • Creating activities and presentations to increase the skills of young people.
