Experienced professional with over 15 years of expertise in various fields that include HIV Program Management, operations and administrative support. I have completed the Community Health Worker (CHW) workforce development program at City College of San Francisco as of May of 2021. During the course of my studies as a returning student, I have had a special focus on HIV prevention and education, particularly among high risk populations. My intention upon entering the field is to leverage the skills I’ve acquired in the CHW program to obtain a position in a community based organization or the public sector that involves HIV patient navigation, prevention and education within the LGBTQ Community. I am also interested in facilitating client referrals as part of an integrated care team to promote health and wellness in the community. The qualifications I have include a strong working knowledge pertaining to the epidemiology, prevention methods, and high risk populations associated with HIV. I have also received training in the areas of motivational interviewing, harm reduction, intake assessment as well as completion of a 4 month internship at Maitri Compassionate Care in San Francisco, California. * Responsible for the overall coordination, project management, and administrative support of the HIV/AIDS care program consisting of a 200+ patient patient population. * Maintains HIV Program quality improvement metrics, policies and procedures in collaboration with the VP of operations, HIV Care Team, and the program’s Medical Director. * Oversees the collection of patient information completes data merging between the HIV client databaseof the State of California (ARIES) and our health center’s EMR (Epic) * Experience working directly with HIV positive patients assisting them with establishing Ryan White program eligibility in completion of annual assessments. * Co-faciilitated HIV Patient Support Groups with other program staff members. * Oversaw successful implementations of multiple workflows specific to patient income and residency requirements, STI screenings, and other continuous quality improvement metrics (CQI) in alignment with HRSA/HAB guidelines as a Ryan White Part C grant recipient.
* Contracted to project manage physical move/relocation of corporate office of approximately 25 full time employees within San Francisco reporting directly to the Director of People & Culture (HR) * Coordinated with multiple vendors via email and phone communications related to office relocation * Conducted physical inventory of all company owned items in advance of office relocation * Set calendar appointments and facilitated in person meetings with external vendors and internal personnel * Conducted on site visits of vendors and master tenant at office to be vacated and new location
