· Examines staff performance and reports to Care Management Leadership, taking remedial action as needed. · Informs RN Care Manager, Social Worker, and clinical team of concerns or delays affecting discharge or post-acute appointment scheduling. · Creates records and reports for clinical reviews and discharge plans for third-party payers and other stakeholders. · Offer phone support for discharge plan and problem-solving post-discharge Reinforce patient/family education on care strategy and outcomes. · Assist with care management contract completion and maintenance during and after patient hospitalization. · Coordinated care with providers for high-risk patients at risk for readmissions 30 days post-discharge. · Direct collaborative outreach to all AdventHealth patients. · Coordinate off-site clinical and physician providers. · Support patients with community resources. · Conduct telephonic and in-person assessments with patients/families. · Record activities and progress in electronic medical records with stated policies and procedures, computer billing/utilization systems, or as required. As needed, enters clinical and discharge planning information into the medical record.
· Manage administrative member intake and admission/discharge information • Process referrals, prior authorizations, and intake. · Coordinate patient appointments with providers. · Conduct telephonic and in-person assessments with patients/families. · Assist RN Care Manager or Social Worker in coordinating care services for assigned patient, collaborating with patient and family. · Verify insurance and DME setup with patients using HIPPA guidelines. • Assist clinical staff with document/triage cases for Clinical Coverage Review. • Address benefit/eligibility, billing, and payment difficulties. · Maintain department goals and productivity • Receive care coordination notifications and give appropriate triage for non-clinical assessments/interventions · Perform clerical tasks such data entry, faxing, answering calls, and communicating with departments.
