Hello all, My name is Jarett, and I am currently a Service Coordinator at Crestwood Behavioral Health. I have found a passion for Mental Health through trials and tribulations with family and myself. I am a dedicated employee and active learner who has growing experience with acute level of care inpatient hospitalizations along with knowledge of community resources that can be uniquely beneficial depending on the needs of clients. I am currently enrolled at San Francisco State University and working towards a bachelors in Business who I transferred from Laney College with an Associates Degree in Social Sciences. My goal is to use my skills and experience to uplift my community through mental health services and community resources.
• Assess clients to identify individual problem areas and coordinate overall treatment plan under the direction of the Program Director and in cooperation with nursing staff • Lead interdisciplinary team meetings to discuss client progress, plan discharges and provide a client-focused space for concerns • Familiarize clients with social services such as Resource Connect and work closely with Crisis Residential Treatment Program (BACS) to ensure continued care of clients • Communicate with room and board, board and care, parole, and local outpatient programs for discharge placement of clients, such as Sober Living Environments, and community shelters • Collaborate with Solano County Hospital Liaison Unit to coordinate care with client families and/or community support system to introduce them to Solano County Behavioral Health
• Delegated tasks, trained and facilitated breaks for general employees. • Organizing & managing closing procedures of the store, closing registers & responsible for cash count. • Excels with creating rapport with guests, but specializes in deescalation if necessary. • Responsible for the critique of negative & positive moments for employees.
