• Direct and oversee services at 2 residential facilities for adults with mental health, chemical health, and housing issues • Develop programming for both facilities and manage budgets to ensure financial viability • Facilitate new staff training and complete presentations and trainings that assist new hires in developing skills to complete paperwork, address client issues, and schedule appointments and time at Fernbrook • Assist clients in obtaining GRH eligibility as a county designee by guiding and advising clients on completing assistance forms including PSN, CAF, Household Report Forms and Interim Assistance Forms • Provide clients with support services (ARMHS) and address concerns with mental health diagnosis pertaining to vocation, independent living, and interpersonal relationships
• Managed a portion of the Shelter Plus Care program which assisted homeless individuals and families in finding housing • Collaborated with multiple agencies and landlords to acquire housing for homeless individuals and assisted individuals within Shelter Plus Care to maintain client homes and paperwork to remain housed • Provided ARMHS services to adults with mental illness and chemical dependency • Assessed clients barriers in dealing with mental health issues and worked on developing skills to manage mental illness • Conducted and completed individual treatment plans to help determine goals related to mental health and other areas of functioning including vocations, interpersonal relationships, and community engagement
• Executed and implemented CTSS and ARMHS services to children and adults with mental health diagnoses • Aided clients in navigating paperwork and services provided by various government agencies. • Developed and implemented treatment plans with clients to increase skills which help rehabilitate clients with mental health concerns and disorders • Assisted clients in completing mental health status forms such as CASII and SDQ which measure current state of mental health and functioning
• Advise and interact with adults and children with various mental and physical health related disabilities • Evaluate and establish behavior plans specific for each client in pursuit of independence
