    Intraoperative strain measurement of ligaments. Soft tissues in human joints such as ligaments, tendons and the capsule both serve a passive mechanical as well as a sensorial function. Up to now, mainly their mechanical function has been taken account of during surgical procedures (e.g. while balancing the knee joint during TKA). Recently, however, more and more orthopaedic surgeons experience that this may not be sufficient for clinical success. Despite the use of ever improved implant designs and more precise surgical techniques, a considerable number of patients (particularly after TKA) remains dissatisfied and suffers from reduced function compared to their healthy peers. This is an indication that the orthopaedics community is missing some important aspect of the working of joints. Mechanical malalignment and component mal-positioning have also been identified as influencing factors for unsatisfactory clinical outcome. Although the impact of neutral mechanical alignment on implant longevity is still a matter of discussion, there is currently no better parameter to aim for when performing TKA. The ligaments of the knee create a truly dynamic bio-mechanism, making ligament tension balancing in TKA surgery extremely challenging. We believe that the solution includes the sensorial role of the soft tissues, particularly the ligaments. The ligaments sense deformation and strain, since they are adept of detecting quite small changes in length and reacting very fast to those by influencing muscle tone. The relatively coarse joint balancing techniques which are in use during orthopaedic surgery are most probably not sufficient to defer to this delicate sensorial function in many patients. Therefore, we need a direct and accurate measurement of length changes of ligaments during surgery in combination with a way to tune implant alignment and/or perform ligament release to keep these length changes within acceptable limits.

    Dr Delport treedt op als gerechtsdeskundige (of justitiearts): dit is bij aanstelling door de rechtbank. Op aanvraag kan het ook als raadsgeneesheer (= raadgevende arts = raadsdokter): de arts-raadsman betreffende de medische besluiten (de "medische advocaat"); de raadsgeneesheer treedt op ofwel voor de partij die de schade moet vergoeden ofwel voor het slachtoffer. Soms treedt Dr Delport ook op als scheidsrechter (of arbiter): deze geneesheer wordt door de twee raadsgeneesheren bij onderling overleg aangesteld bij de aanvang van de minnelijke medische expertise (afgekort "M.M.E."); hij dient in feite de taak van een gerechtsdeskundige uit te voeren voor het geval de 2 raadsgeneesheren niet tot de nodige overeenstemming zouden komen; in een dergelijk geval maakt hij dus autonoom de medische besluiten op, binnen het kader van de M.M.E.
