Here, I spearheaded the transformation of a pre-existing case management program into a flagship Care Coordination initiative within the New York State Department of Health’s Health Home model. My efforts resulted in exceptional Agency representation across various local, municipal, and State committees and sub-committees. I took the lead in authoring a highly esteemed Policy and Procedure manual, alongside workflows and scripts, which were embraced by multiple governing Health Homes and adapted into regulatory policies and best practices. One of my significant achievements was expanding the program from twelve to sixty-four, overseeing eight direct reports, and catering to over 1,700 members. I managed chronic diseases across medical and behavioral health care continuums, ensuring comprehensive care for all individuals involved. I also took charge of conceptualizing, designing, and implementing a proprietary data management solution. This solution was vital in ensuring regulatory compliance in reporting and information storage, providing a streamlined and efficient approach to managing critical data.
In my role as a Senior Reentry Care Coordinator at Spectrum Health & Human Services in Buffalo, New York, I led a team of care coordinators to provide essential support for individuals transitioning from incarceration to community reintegration. By overseeing the coordination of vital services and partnering with the New York State Department of Corrections and Community Services and the New York State Offices of Alcoholism and Substance Abuse Services as well as community based organizations, our team promoted successful reentry and improved outcomes for program participants.
As a Reentry Care Coordinator I supported a caseload of forty incarcerated persons transitioning from a medium security State Correctional Center to New York State Parole and community reintegration. I provided comprehensive assessment and diagnostic support for substance use related disorders, developed collaborative plans of care in partnership with New York State regulatory partners, program participants and community based organizations, and provided in-person as well as telephonic support for served persons both while incarcerated and upon return to the community.
