- Assess patients at the Institute of Mental Health (IMH) and provide consultation to Medical Social Workers on the patients' most suitable care options. - Monitoring of referral trends and KPIs, reporting them to internal and external stakeholders. - Organised Value Stream Mapping workshops with Nursing Homes (NHs) and IMH to (1) decrease admission turnaround time to NHs, and (2) foster partnerships and build capabilities. - Collaborated with other AIC departments to enhance and standardise the existing assessment tool used for MOH subvention claims. - Coordinated the assignment of referrals on the national waitlist to all NHs. Subsequently, collaborated with several departments to set up an online referral assignment system that automated this function. - Translated data and customised reports for internal and external stakeholders. Automated these reports in Qlikview / Excel. - Represented the department in an interdepartmental project to set up AIConnect, a resource page for service providers that consists of contact lists, networking events, and bird eye's view of AIC's functions and projects.
