Assess nutritional status and implement nutrition focused interventions, including TPN and EN management, for 10 to 14 patients daily in the medical/surgical ICU and surgical floors to optimize nutrition provisions and clinical status of patients. Additional experience as primary coverage for oncology, inpatient rehab, and medical/surgical floors. Manage PN prescriptions and engage with TPN pharmacy and medical team daily to discuss adjustments to PN order with the goal of optimizing nutrition care for patients. Attend critical care rounds weekly as a collaborative member of the interdisciplinary team. Provide in depth nutrition education for patients throughout their admission to promote general health and optimize their clinical outcome, including post-ostomy, heart healthy, diabetes diet education. Lead and develop training video and annual in person nutrition education sessions for medical and surgical residents and ICU nursing staff covering various topics including parenteral nutrition, enteral nutrition, and malnutrition. Active member of the critical care committee and malnutrition workgroup. Assisted with IDDSI implementation at hospital site, including food testing and categorizing. Support the growth and development of interns and coworkers through precepting and conducting in service training.
Review 5-10 potential continuing education courses annually to ensure that the presentations are accurate, credible, and informative prior to publication for various companies including Fresh Avocados- Love One Today.
Boston, MA Integral member of an interdisciplinary team working to optimize the health of low income adults 55 years and older with the overall goal of promoting and supporting independence. Monitor and enhance the nutrition status of more than 260 PACE participants through initial and annual clinical assessments, nutrition counseling, and supportive services when appropriate including grocery delivery service and meals on wheels. Develop, implement, and adjust nutrition care plans for high risk patients including initiating supplement regimens, feeding schedules, appropriate meal services, enteral feeding regimens, and nutrition counseling. Oversee daily lunch service at PACE center to ensure compliance with food safety standards and participant diet orders. Manage and oversee the daily job performance of diet aide including lunch service, patient care, kitchen cleanliness, and product restocking and inventory.
