Performs ongoing systematic collection and analysis of patient data pre – during – post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader Charge Nurse Supervisor or Physician as needed.
Evaluate patients for hospice appropriateness Educate loved ones on changes in status Interprofessional collaboration to ensure comfort, spiritual, medical, and emotional needs are met
Provide patient care, perform assessments and interventions on respiratory, neurological, orthopedic, cardiac, oncological and other critically unstable patients Assess patients’ condition, notify physicians of clinical changes as well as other members of the interdisciplinary team including nurses, rehab, respiratory, wound care, and case management Monitor vital signs, adequacy of ventilation, and levels of consciousness and pain Administer medications/ Titrate drips as determined by the patient’s condition
