Senior mental health practitioner offering comprehensive assessments, clinical supervision and critical MDT discussions. Developing robust care plans spanning the bredth of nursing including LD, Mental health and physical health. The main part of the role was case managing young people, offering therapeutic support and supporting young people in mental health struggles. Assessments were completed and escalated or signposted when appropriate.
The role consisted of running a mobile, static and ad hoc depot clinic with over 150 active patients, this included medication management, ordering assessment of current mental health state, escalation when needed. MDT working. Operating a 30+ caseload of complex presentations including schizophrenia, depression, personality disorder, anxiety, prsd, LD presentations. Assessing, management and signposting for people.in the community.
