In my role as a Special Education Teacher at Brevard Public Schools, I successfully wrote and implemented individualized education plans (IEPs) for students with diverse learning needs. Through collaboration with teachers and staff, I ensured a supportive and inclusive classroom environment, leading to improved academic and social outcomes for students. • Developed and implemented personalized education plans to support student learning and growth. • Collaborated with colleagues to create a supportive and inclusive classroom environment. • Utilized innovative teaching strategies to help students achieve academic and social goals.
Working in this setting, we provide comprehensive social work services to inpatient clients and their families. As a department, we are tasked with assessment, discharge planning, case management, documentation in an inpatient psychiatric setting. Our Social Work Department work alongside various disciplines to develop treatment plan goals and interventions tailored to each client’s specific needs. In this position, I have the opportunity to work with a diverse population, and assist with facilitating appropriate placement as needed, follow up care, and resources upon discharge. These pivotal roles and responsibilities have allowed me to create and maintain close working relationships with outside agencies and facilities, while ensuring our clients’ have appropriate support in place upon discharge from our facility.
Healthy Start Program funded by Florida Department of Health-Responsible for enrolling prenatal women and their families into the Healthy Start Program and providing ongoing support, case management services and care coordination services, health education, and referrals to link Healthy Start participants to community resources.
Provided direct care to ensure the safety and well-being for adolescent patients with behavioral problems and/or psychiatric disorders.
