Evaluate and treat patients with neurological and psychological disorders, stroke, orthopedic injuries, and SCI, COPD, accurately documenting patient’s functional impairments and capabilities in the electronic medical record system, set length of stay and goals for individual patients, and implemented an interdisciplinary approach with low level patients for better recovery while maintain patient safety.
Evaluated and treated patients with orthopedic injuries, neurological disorders, stroke, generalize weakness, MS, CHF, COPD, and SCI, implemented interdisciplinary approach with Nurse Practitioners and Doctors on staff along with MDS’, performed managerial tasks, implemented various programs to improve quality of care to patients (lymphedema program, outreach program, and contracture program), accurately documented patient’s functional impairments and capabilities, set length of stay and goals for individual patients, accurately and efficiently documented patient’s progress towards goals, treatment plan, and discharge, implemented an interdisciplinary approach with low level patients for better recovery, assisted in in-services to train CNAs on proper lifting and transferring techniques to decrease injuries, and accurately document progress toward goals, treatment plan, and discharge.
