This role has me provide clinical support within a multidisciplinary allied health team, delivering person-centred holistic care across community-based transitional aged care programs while working under the delegation and supervision of allied health professionals to implement therapy interventions and prepare clinical environments for individual sessions. Support continuity of care for older persons by facilitating rehabilitation and home-based therapy in Mercy Health’s Transitional Aged Care Program.
In this role, I work with diverse individuals who have experienced traumatic brain injuries within a rehabilitation setting. My role is supporting their individual goals from their injuries and medical impairments that impacted their physical, mental, or cognitive abilities. I work with closely with individual clients, under the guidance of Allied Health Professionals to improve memory, decision making, communication, problem-solving skills and more, all aimed at enhancing their overall quality of life.
This role has me delivering direct support in speech pathology and occupational therapy under the supervision of registered therapists—serving children, adolescents, and young adults through in-clinic and telehealth settings. Assisting clients in building communication and functional life skills, supporting respectful, empowering, and inclusive care consistent with Interact Health Group’s mission to help clients achieve their potential. Also, apart of my role has me planning and facilitating holiday groups to cater to individual goals while keeping in my scope of practice.
In this role, I assist participants both individually and in group settings, focusing on enhancing communication skills and achieving personalized goals in their daily lives. I work within natural environments, engaging in early intervention and following plans prepared by Allied Health Professionals. My responsibilities include completing delegated tasks, facilitating group activities, and creating individualized resources, all while collaborating closely with a multidisciplinary team to support each participant's plan and goals.
