Provide support to clients, family members, and/or guardians/conservators regarding developmental disabilities, services, and supports to assist the client/family to achieve their desired goals and objectives. Collaborate with and/or consult with agency or external multi-disciplinary resources to assist clients in identifying their needs and determining plans to meet their needs. Provide advocacy as appropriate to ensure the rights of the client on the caseload are fully protected including, legal, civil, and administrative rights to persons with developmental disabilities
Support and provide support and crisis intervention through trauma-informed care. Ensure the privacy and safety of occupants by not confirming identity of other guests, and by not allowing anyone other than occupants, staff, case workers or law enforcement on the premises. Complete shadowing tasks as assigned when new team members join the staff team. Maintain positive, supportive and respectful occupant relations.
Facilitation of activities to meet residents spiritual, social, physical, and emotional needs. Created activities full of fun and creativity! Responsible for planning and implementing programs of exercise, education, spiritual, and community outings.
