Worked with Behavioral Health Consult Team to assist in crisis assessments, patient care, and discharge planning for hospital patients with mental health needs. Documented encounters with clients in electronic medical record, including full assessments and mental status exams. Made inpatient and outpatient referrals for patients based on their care needs.
