Provide continuous assessment of patient and family’s needs and implementation of the interdisciplinary plan of care in an inpatient setting. Evaluate patient’s clinical condition to determine medical necessity for inpatient level of care in compliance with Medicare guidelines. Coordinates necessary arrangements for the patient to receive continuous, alternative care or to be transferred to appropriate level of care in a timely, cost-effective manner.
