· Investigate whether a child is in need of protection, while adhering to the Child and Family Services Act · Develop and implement comprehensive case plans to increase the level of safety for children within the family unit · Assess and monitor the social, emotional, and developmental functioning of children · Assess family functioning and parenting capacity of caregivers · Provide short term and long term care treatment plans of children when admitted into care, and determine when a child is able to be safely reintegrated back to the family · Communicate with professionals from different institutions such as those from the judicial system, law enforcement, the education system and the health care system, to ensure the well-being of children and families · Refer individuals to counselling programs and resources geared to meet their needs
· Responsible for providing crisis intervention counseling to women residing in the shelter and on the crisis line · Provide advocacy and referrals to women leaving the shelter and going into the community and to women who called the crisis · Ensure that all necessary resident files, forms and documentation are complete · Responsible for the day to day running of the shelter · Support women in safety planning for themselves and their children before, during and after separation
• Conduct telephone intake calls and in-person formal intake interviews • Provide case management to clients such as keep up-to-date and accurate documentation regarding all clients interaction, support clients in goal setting/achievement, assist clients with forms/documents necessary for housing, legal aid, child welfare concerns, employment and act as advocate for clients involved with other agencies/services • Provide crisis intervention • Provide one-on-one and group counseling with clients
• Provide frontline support, advocacy, counselling and behaviour management services • Develop and implemented life skills and diversity programs • Participate in team discussions/meetings with the focus of patient/client welfare • Provide 1:1 support for several clients with special needs both Residential and School based • Maintain safe, therapeutic and organized living environment • Ability to work in an anti oppressive practice to support client/patient • Carries out lesson plans, classroom routines and/or reinforces previously taught concepts as directed by the Teacher
Provided reintegration services for youth offenders as such as: helped with the Transitional School Program (Partnership with TDSB), E.P.A. at the Roy McMurtry Youth Detention Centre, Housing services, and Employment services. Was also responsible for : • Student intake assessments • Plan extra-curricular activities • Writing donation grant proposals
