Health is impacted by more than just clinical performance. Social and economic factors are estimated to attribute up to 40% of an individual’s health, creating a rising consumer need for bundled community and social support services. This has been the impetus of UnitedHealthcare myConnections™, a first-to-market solution powered by a single integrated platform that enables a better way for low-income Medicaid and vulnerable populations to participate across a broader range of government-funded and community services. As a state Executive Director, Adam developed strategies linking resources and cultivating relationships to help local communities increase health outcomes by integrating innovative programs that address the social determinants of health. As a national consultant, Adam operationalized projects that targeted frequent utilizers of managed care health services, coached provider groups such as FQHCs, developed training modules that implemented whole person care models and lead efforts to screen and serve victims of Intimate Partner Violence (domestic violence).

Objectives included launching a campaign in support of facility and service expansion, re-branding five practices under one umbrella, and establishing a sustainable organizational structure. Responsibilities included serving on the leadership team, launching new service initiatives and facilitating community relationships.
